Mandi Diamond, PCMH CCE brings to DataGen, Inc. over 15 years of practice transformation experience and 25 years of hands-on clinical project management and information technology (IT) knowledge. Since joining HANYS in 2015, Ms. Diamond has guided countless practices nationwide of varying sizes, specialties, and organizational structures through their Patient-Centered Medical Home (PCMH) transformation journey. She also consults with practices on value-based initiatives and other National Committee for Quality Assurance (NCQA) recognition and accreditation programs, such as Patient-Centered Specialty Care (PCSP), and Health Equity Accreditation (HEA).
